Approved GLP-1 pills are broadly comparable to standard-dose GLP-1 injections for weight loss, but no direct head-to-head trial has shown that they are equal. Oral Wegovy 25 mg produced an average weight loss of 13.6% after 64 weeks. Injectable Wegovy 2.4 mg produced an average loss of 14.9% after 68 weeks in a separate trial. Because the participants and study designs differed, these results cannot prove that the tablet works exactly as well as the injection.
Injections have the longer track record, especially for people who want the strongest established evidence, simpler dosing or the option of a higher approved dose. Oral Wegovy may suit adults who want to avoid needles and can follow its daily timing instructions.
Oral GLP-1 Pills and Injections Compared
| Factor | Oral Wegovy tablet | Injectable Wegovy |
|---|---|---|
| Active ingredient | Semaglutide | Semaglutide |
| Typical weight-loss dose | 25 mg once daily | 2.4 mg once weekly; 7.2 mg injection is also available for some adults |
| Average result in major trial | 13.6% weight loss at 64 weeks | 14.9% at 68 weeks with 2.4 mg |
| Dosing routine | Take in the morning on an empty stomach with up to 4 ounces of water. Wait at least 30 minutes before eating, drinking or taking other oral medicines. | Inject once weekly, with or without food, at any time of day |
| Direct pill-versus-injection trial | Not established | Not established |
| Main advantage | No needle | Weekly dosing and a longer clinical track record |
| Main limitation | Daily fasting and timing requirements | Requires a weekly injection |
The doses cannot be compared milligram for milligram. Oral semaglutide has limited and more variable absorption through the stomach, so the tablet uses a much larger milligram dose than the injection.
How Much Weight Can Oral GLP-1 Medication Produce?
Oral Wegovy produced an average weight loss of 13.6% after 64 weeks in the OASIS 4 trial. The placebo group lost 2.4% of its starting body weight.
Among participants taking oral semaglutide 25 mg:
- 76.3% lost at least 5% of their body weight
- 59.8% lost at least 10%
- 47% lost at least 15%
- 27.9% lost at least 20%
For comparison, adults in the STEP 1 trial who used injectable semaglutide 2.4 mg lost an average of 14.9% after 68 weeks, compared with 2.4% in the placebo group.
The results place oral Wegovy and standard-dose injectable Wegovy in a similar range. They do not establish that the two treatments are equally effective because they were not tested against each other in the same randomized trial. The FDA has also stated that there is no evidence showing that Wegovy tablets are superior to other approved GLP-1 treatments for weight loss.
Are Higher-Dose Injections More Effective Than GLP-1 Pills?
Higher-dose injections may produce more weight loss than oral GLP-1 medication, but the evidence comes from separate trials.
In one trial, injectable Wegovy 7.2 mg produced an average weight loss of 18.8% after 72 weeks in adults with obesity and no type 2 diabetes. The injectable 2.4 mg group lost 15.5% in that study.
Those results cannot be compared directly with the 13.6% result for oral Wegovy because the trials had different participants and designs. Still, the available evidence does not show that the tablet produces greater weight loss than higher-dose injections.
If maximum average weight loss is the main goal, a clinician may consider an injectable option, including semaglutide or tirzepatide, when medically appropriate.
What Is the Difference Between Wegovy Tablets and Rybelsus?
Wegovy tablets and Rybelsus are not interchangeable.
- Wegovy tablets contain oral semaglutide 25 mg and are approved for chronic weight management in eligible adults with obesity or overweight plus a weight-related condition.
- Rybelsus contains oral semaglutide and is approved to improve blood sugar control in adults with type 2 diabetes. Its FDA indication is different from the chronic weight-management indication for Wegovy tablets.
Rybelsus should not automatically be treated as an equivalent weight-loss substitute for Wegovy tablets or injectable Wegovy. The products have different approved dosing schedules and indications.
Which Is Easier to Take?
Injectable Wegovy is easier to schedule. It is taken once weekly and can be administered at any time, with or without meals.
Wegovy tablets must be taken correctly to improve absorption:
- Take the tablet in the morning on an empty stomach.
- Use no more than 4 ounces of plain water.
- Swallow the tablet whole.
- Wait at least 30 minutes before eating, drinking other beverages or taking other oral medications.
The tablet may be harder to fit into a morning routine if you take other medications, eat soon after waking or cannot maintain the same schedule every day.
Do the Pill and Injection Have the Same Side Effects?
Both forms can cause gastrointestinal side effects, particularly during dose escalation. Common problems include nausea, vomiting, diarrhea, constipation, abdominal discomfort and reduced appetite.
In the oral semaglutide obesity trial, gastrointestinal adverse events occurred in 74% of participants taking the tablet, compared with 42.2% of those receiving placebo.
Both oral and injectable Wegovy carry a boxed warning about thyroid C-cell tumors observed in rodents. Wegovy is contraindicated for people with a personal or family history of medullary thyroid carcinoma or with Multiple Endocrine Neoplasia syndrome type 2.
The products should not be combined with other semaglutide products or another GLP-1 receptor agonist unless a qualified clinician specifically directs it.
Are Compounded GLP-1 Pills as Effective?
There is no reliable basis for assuming that a compounded oral semaglutide product is as effective or safe as an FDA-approved tablet.
The FDA does not review compounded drugs for safety, effectiveness or quality before they are marketed. It has also warned against marketing compounded GLP-1 products as interchangeable with approved medicines.
Bottom Line
The tablet may be a good fit when avoiding needles matters and you can follow the daily fasting routine. Injectable Wegovy may be a better fit when you want weekly dosing, a longer evidence base or the possibility of greater weight loss with a higher approved dose.
Do not assume that Rybelsus, compounded semaglutide or an online "GLP-1 pill" is equivalent to prescription Wegovy tablets or injectable Wegovy.
The choice depends on your diagnosis, other medications, gastrointestinal history, insurance coverage, ability to follow the dosing instructions and weight-loss target.
