ATTAIN-1 is the largest published trial of an oral GLP-1 for weight

ATTAIN-1 enrolled 3,127 adults with obesity and followed them for 72 weeks on orforglipron. Mean weight loss was 11.2 percent of body weight. Just over half the participants, 54.6 percent, lost at least 10 percent.

Read the average carefully. A mean of 11.2 percent describes the group, not any individual. Within that average some people lost a great deal more and some lost very little.

Switching from an injection held the result

A separate phase 3 trial looked at people already taking an injectable incretin who moved across to orforglipron. Those who transitioned from semaglutide maintained the weight they had lost. That matters for anyone tolerating an injection well but wanting to stop injecting, and it is the question most people switching actually have.

Cardiometabolic markers moved alongside weight

Pooled analyses of orforglipron trials reported improvements in cardiometabolic measures rather than weight alone. These are intermediate markers. They are not the same as a demonstrated reduction in heart attacks or strokes, which takes dedicated outcome trials running for years.

The limits worth holding on to

Trial populations are selected. Participants are monitored, supported and followed up in ways ordinary care rarely matches, so real world results tend to be more modest. Trials also run for a fixed period: 72 weeks tells you nothing about year five.
