Why Tirzepatide Is Taken Once Weekly
The FDA prescribing information for Mounjaro and Zepbound specifies once-weekly administration. The maximum approved adult dose is 15 mg once weekly. Tirzepatide has an approximate half-life of five days, so it remains in the body for several days after an injection.
Injecting sooner than scheduled can result in more medication in your body during the same seven-day period. That may raise the risk of nausea, vomiting, diarrhea, constipation, abdominal pain, dehydration, and other side effects.
Lilly states that dividing Mounjaro into smaller or multiple doses has not been studied. There is no available safety or effectiveness information for that approach.
What if Tirzepatide Seems to Wear Off Before the End of the Week?
Do not add a second injection on your own. Ask your prescribing clinician to review:
- Your current tirzepatide dose
- How long you have taken that dose
- Your appetite, blood glucose, or weight response
- Any side effects or gastrointestinal symptoms
- Other medicines, including insulin or sulfonylureas
The FDA label recommends choosing a maintenance dose based on treatment response and tolerability. If you cannot tolerate a maintenance dose, the label allows consideration of a lower maintenance dose. That is different from splitting the dose without medical guidance.
Changing Your Injection Day Is Different From Dosing Twice Weekly
Mounjaro and Zepbound allow you to change your weekly injection day if there are at least 72 hours between doses. This is a schedule change, not permission to inject twice every week.

Do not create your own twice-weekly schedule from a compounded vial. Compounded tirzepatide is not FDA-approved, and the FDA has reported dosing errors involving incorrect measurements, incorrect concentrations, more frequent dosing, and doses above those in the FDA-approved label.

- Trouble breathing
- Swelling of the face or throat
